Thank you for your interest in InSight Vision Therapy, where our goal is to maximize your vision potential. The first step in this process is a vision and eye health evaluation. A new patient examination will likely take an hour, as Dr. Carter takes a thorough history of your vision in addition to your overall health and wellness. As part of this process, she will do many different tests of your visual abilities, eye-teaming, and eye health in order to accurately assess your needs.

After scheduling an appointment at our office, we will email you a link for our online forms. In addition, please review the New Patient Forms below. Print, fill out and bring to your appointment the forms that are relevant to you.

Our office is out of network with all insurance plans. Most plans do allow us to bill for the appointment, but Medicare and Medicaid do not. If you have one of these insurance plans, please print and fill out the appropriate out-of-network form prior to your appointment:

Payment Methods

Our office takes cash, personal checks and credit cards: Visa, Mastercard, Discover and American Express. We also offer Care Credit, which may allow patients more time to pay their balance. In addition, patients may utilize their Health Savings Accounts (HSAs) or Flex Spending Accounts (FSAs) to pay for their examinations and vision therapy.
